What Is antibiotic stewardship?
Antimicrobial stewardship is a coordinated program that promotes the appropriate us of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
Misuse and overuse of antimicrobials is one of the world’s most pressing public health problems. Infectious organisms adapt to the antimicrobials designed to kill them, making the drugs ineffective. People infected with antimicrobial-resistant organisms are more likely to have longer, more expensive hospital stays, and may be more likely to die as a result of an infection.

The goal of antibiotic stewardship is to monitor and enhance patient and physician use of antibiotics. To successfully treat infections, safeguard patients from the risks associated with unneeded antibiotic usage, and fight antibiotic resistance, it is imperative to improve antibiotic prescribing and use.Providers and facilities can use the Core Elements of Antibiotic Stewardship as a set of fundamental guidelines to improve antibiotic use, promote patient safety, and enhance results.

Due to the complexity of C. difficile infection prevention, stewardship programmes have the chance to work in conjunction with other organisations like infection prevention and the laboratory.Resistance to antibiotics.

Studies evaluating the impact of increased antibiotic use on resistance rates have shown conflicting results indicating that the emergence and spread of antibiotic resistance is multifactorial.
The easiest way to determine how hospital stewardship interventions affect resistance is to measure microorganisms that are recovered from patients after admission while they are subject to those interventions. It has also been proven to be helpful to track resistance at the patient level, namely the percentage of patients that develop resistant superinfections. The NHSN Antimicrobial Resistance (AR) allows hospitals to monitor antibiotic resistance as well.

Outpatient settings is an important component in antibiotic stewardship. These are the important steps involved in the outpatient stewardship practice. Physicians can educate patients to follow at least one of the following:
Use effective communication strategies to educate patients about when antibiotics needed are not
Educate patients about the potential harms of antibiotic treatment
Provide effective patient counselling
Provide. Continuing education activities for health care professionals

Why is Antibiotic Stewardship Important for Nursing Homes?
Some of the most often recommended medications in nursing homes are antibiotics. Up to 70% of nursing home residents receive an antibiotic within the course of a year. Between 40% and 75% of antibiotic prescriptions are made in error.Antibiotics are frequently recommended in nursing homes to prevent respiratory tract infections (RTI) and urinary tract infections (UTI). Antibiotics are frequently misused when prescribed before an infection has even occurred.Residents are frequently prescribed antibiotics merely because they are colonised by (carrying) microorganisms that do not cause illness. Antibiotic misuse is facilitated by the prescription of antibiotics for colonisation. By following the policies and procedures, antibiotic use can be reduced.Implementing policies and processes in nursing homes can significantly enhance the utilisation .some of the ways are

- Policies which support the maximum use of antibiotic
- Broad interventions to improve antibiotic use
- Pharmacy interventions to improve antibiotic use
- Tracking and reporting antibiotic use and outcomes.
- Proper education to the health care professionals.
