Acetylcholine: What You Need to Know

Acetylcholine (Ach) is a neurotransmitter that works both in the central and peripheral nervous systems. The first neurotransmitter to be identified as acetylcholine.  Imbalances in acetylcholine levels have been linked to chronic conditions such as Alzheimer's and Parkinson's. 


Acetylcholine and Alzheimer’s disease:


Alzheimer's disease is the major cause of dementia among the elderly. Significant memory loss and cognitive issues that impede day-to-day functioning are some of its symptoms. Alzheimer's disease has no known cure. Experts are unsure what causes Alzheimer's disease. However, they are aware that many individuals with the illness have decreased acetylcholine levels. The cells that produce and utilize acetylcholine are damaged in Alzheimer's disease.


 Certain drugs can cause an increase in acetylcholine levels. They accomplish this by inhibiting the activity of enzymes that degrade the neurotransmitter. Acetylcholinesterase (AChE) is the major enzyme in this class, and medicines that reduce the activity of these enzymes are known as AChE inhibitors or cholinesterase inhibitors. AChE inhibitors help with symptoms that are connected to cognitive functions like language, judgment, and memory.


AChE inhibitors example: donepezil, galantamine, rivastigmine 


Acetylcholine and myasthenia gravis:


Myasthenia gravis is an autoimmune disorder characterized by muscle weakness. The immune system blocks or destroys acetylcholine receptors as a result of myasthenia gravis. The muscles are then deprived of the neurotransmitter and are unable to function normally. Specifically, muscles cannot contract without acetylcholine. 


Myasthenia gravis symptoms can be moderate to severe. They include weakness in the hands, fingers, arms, legs, or neck, one or even both eyelids dropping, double or distorted eyesight, difficulty swallowing, breathing difficulty, and speaking difficulties.


Many individuals with myasthenia gravis can lead normal lives. A variety of treatments are available to manage symptoms. AChE inhibitors, which are used to manage Alzheimer's disease, may also help alleviate symptoms of myasthenia gravis. These medications delay the breakdown of acetylcholine. By increasing the quantity of acetylcholine that is accessible, these acetylcholinesterase inhibitors aid in the activation and contraction of muscles. 


Acetylcholine and Parkinson’s disease:


Parkinson's disease is a neurodegenerative disorder that involves unintentional movements, tremors, and cognitive and mood problems. For effective movement control, the body needs a balance between the chemical messenger acetylcholine and dopamine. The precise causes of Parkinson's disease remain uncertain. Experts have found that individuals with the illness frequently experience a decline in dopamine that enables acetylcholine to take control. This results in symptoms like jerking motions and tremors because the muscles get overly "stimulated”. 


As a result, several Parkinson's disease medications inhibit the function of acetylcholine. This enables a rebalancing of dopamine levels, which may help with some symptoms. These drugs are referred to as anticholinergics. Anticholinergic drugs function by inhibiting the receptors (muscarinic receptors) for the neurotransmitter acetylcholine. They can also help with dyskinesias, which are exaggerated movements that can be caused by other Parkinson's drugs.


Botox and acetylcholine:


Botulinum toxin also referred to as Botox, is used to treat several muscle-related diseases. Additionally, Botox injections can help with certain bladder and bowel problems, excessive sweating, and migraine headaches.


Furthermore, Botox is the most widely used nonsurgical cosmetic procedure in the United States. Botox works primarily by interacting with acetylcholine in the affected muscle. Because Botox keeps muscles from contracting, injecting it into particular facial muscles, for instance, can result in a temporary decline in wrinkles. The skin over the muscle seems smoother as a result.
